Use the secure form below to refer a client. Self-referrals are welcome — first-time clients don't need an existing diagnostic assessment. We'll respond within one business day.

Eligibility
Who qualifies?
ARMHS is available to adults in Minnesota who meet the following:
- Ages 18 or older
- A mental health condition that causes substantial functional impairment in three or more areas of daily life
- Cognitive capacity to benefit from rehabilitative services
- Medical Assistance (MA) or a Prepaid Medical Assistance Plan (PMAP)
First-time client? You don't need a prior diagnostic assessment. If one isn't in place, we'll coordinate it as part of getting services started.
What happens next
Our referral process
- Submit the form — basic client and referrer information, plus any clinical context you can share.
- We reach out within one business day — to confirm receipt and discuss next steps.
- Intake & assessment — we coordinate the diagnostic assessment (if needed) and the functional assessment.
- Services begin — a treatment plan is built with the client, and ARMHS services start.

Existing Diagnostic Assessment (optional)
If the client already has a current DA from a qualified mental health professional, we can work from it. Otherwise we'll handle it.
Release of Information (if referring)
A signed ROI lets us coordinate with existing providers, family, and community supports as needed.
